Here’s what you will need to do:
Complete our registration form
Simply download our form and complete your details. Once your form is complete, please return to us by either hand, post or email: Practice@StMartinSurgery.gpnet.je
We will also need to see some supporting documents:
Photo ID
(passport, driving licence)
Proof of Address
(utility bill)
Health Social Security Card
Please note, if you have submitted your form by email or post, we will need to see the original copies of the above documents when you attend the surgery
Your First Appointment
Once we have received your completed registration form, our Admin Team will review and process the documents. You will then be contacted and invited to book a ‘New Patient Appointment’. This is where the Doctor will review your medical history and get to know both you, and your healthcare needs. Please note that the ‘New Patient Appointment’ must be with either Dr Albert or Dr Terry.
Visitors to Jersey
If you are just visiting Jersey temporarily then please download and complete a Visitors Registration form (PDF). Please be aware that payment in full is expected when you attend the surgery, or when the doctor visits you in the community.
Visit St Martin Pharmacy
The onsite pharmacy is available to dispense prescriptions, as well as stocking a wide range of healthcare products.
